El-Saharty joined the Bank in and was the first Egyptian to work in the health, population, and nutrition sector at the Bank since its establishment. At the same time, while substantial gains have been made in the battle against infectious diseases and child and maternal mortality, these remain a major concern, resulting in a double burden of disease. And the country has only one-tenth of the number of radiotherapy centres recommended by the WHO based on its population size and burden of disease. Moreover, some crude forms are produced and used by the poor, usually by fermenting boiled rice, sugarcane, and molasses. The lack of information on the prevalence of CVD restricts health professionals and policy makers from realizing the magnitude of the problem in Bangladesh. El-Saharty provided policy advice and managed programs and projects that spanned a wide range of health programs and systems including maternal and child health, nutrition, communicable and non-communicable diseases, health promotion and behavior change, health service delivery, hospital management, human resources, health governance, public expenditures, health financing, health insurance, and health reform.

He is one of the leading experts who designed the Health Sector Reform Program in Egypt in that piloted the expansion of health insurance coverage and introduced the family health model in the country.
